According to the The Australian Diabetes, Obesity and Lifestyle (AusDiab) study, more than 100,000 adult Australians are diagnosed with diabetes each year. Diabetes can affect a person's quality of life both in the present and for their entire future, impacting on their physical and psychological health, occupational capacity and relationships, among other aspects of life. Many recent studies have demonstrated benefits for people with diabetes by improving self-management and behaviour change.
In this Short Course you will learn about several evidence-based strategies that can be integrated into day-to-day general practice to assist you in optimising care of people with chronic diseases including diabetes. As you work through this Short Course, please consider many of your patients with diabetes and reflect on how you might utilise your learning to change (or confirm) your current practice. During the Short Course there will be several discussion areas where you will be encouraged to take advantage of the opportunity to discuss your views, experiences and any dilemmas with your colleagues.
Topics included:
How to select appropriate patients for self-management
How to successfully empower patients through education, effective communication, and expectation management
Tools for collaborative planning
How to provide ongoing support for behavioural change
The role of GP motivational interviewing during consultations in facilitating behaviour change
Insight into optimizing your practice structure and culture to create an environment that fosters coordination, cooperation, and communication.
